This is to notify the Medical Officer of Health of my intentions to reopen a recreational camp in accordance with O. Reg. 503/17: Recreational Camps, under Health Protection and Promotion Act, RSO 1990, c. H.7

As the person intending to operate the camp, I understand that I will also be considered the operator, unless I designate another individual as the operator at this time.
