Effective July 1, 2018, all operators intending to renovate or reconstruct a personal service setting in Ontario must notify the local Public Health Unit of their intent, as per subsection 3(4) of the Personal Service Settings Regulation 136/18 made under the Health Protection and Promotion Act R.S.O. 1990 Ch.7.
Renovation does not include routine maintenance or repair or replacement of existing equipment.
Please be advised, the completed form must be submitted to Public Health at least 14 days before commencing the construction or renovation.
