Effective July 1, 2018, all operators intending to operate a personal service setting in Ontario must notify the local Public Health Unit of their intent, as per subsection 3(1) of the Personal Service Settings Regulation 136/18 made under the Health Protection and Promotion Act R.S.O. 1990 Ch.7.
Please be advised, the completed form must be submitted to South East Health Unit at least 14 days before commencing operation.
